小唯 A06 c140c9c956 fix(model-health.py v3): 排名驱动自动替换 + 探针3次平均 + 修复致命缺陷 
- 修复: 旧版只修providers.models列表, 从不验证model.default字段
  → prof-b被写成NewAPI不存在的模型全挂503
- 修复: 排名公式丢失context_score, 1M长上下文模型排不上
- 新增: _verify_model_usable 替换前真实调用验证(HTTP200+有内容)
- 新增: 排名驱动升级 - 每次巡检检查default_model是否排名第一, 不是就自动换
  (仅newapi-local免费模型, 付费模型保护)
- 新增: 质量探针每题测3次取平均, 消除单次波动
- 新增: OpenClaw fallbacks修复 + 死模型清理
- 实测闭环通过: 故意改坏prof-b配置→脚本自动修复为nemotron-3-super
